These Terms and Conditions apply to the use of the Physio Health Perth website and services. By accessing our website or engaging our services, you agree to these terms.
Services
Physio Health Perth provides:
Adult and Paediatric Physiotherapy
Hydrotherapy
Manual Handling Assessments
Mobility Assistive Technology (AT) Assessments
Falls Assessment and Intervention
Services may be funded privately or through the NDIS, including categories such as Improved Daily Living and Early Childhood Intervention.
Appointments and Referrals
Appointments are scheduled after referral processing and are subject to clinician availability. We reserve the right to decline or discontinue services where appropriate.
Cancellations and Fees
Appointments must be changed or cancelled at least two (2) business days in advance
Late cancellations or non-attendance may incur a 100% cancellation fee, in line with NDIS guidelines
Travel time and mileage may be charged according to the current NDIS Price Guide
NDIS Responsibilities
Participants are responsible for:
Ensuring sufficient NDIS funding
Confirming service management type (NDIA-managed, plan-managed, or self-managed)
Providing accurate and current NDIS details
Physio Health Perth is not responsible for funding shortfalls or rejected claims.
